Insurance CPT codes

I just started taking insurance for massage therapy.  For those of you with processing claims, do you know if it is standard to include a charge for initial evaluation codes (i.e., 99201-03 for new patients and 99211-13 for existing patients) in addition to the 99124/99140 codes?  Thanks!
